7.     Current year School going proof in case of student.

 Pan Card, Passport, Election Card, Collage I C, Employer I D, Driving License, Copy of Bank Pass Book with Photo.

    ADDRESS PROOF

Ration Card, Passport, Driving License, Electricity Bill, Phone/Cell Phone Bill, Bank Pass Book, House Agreement, Leave & License Agreement, Employer Certificate. (Address of the Address Proof should be mention in any one column of proposal form i.e. Q. No: 1 OR in Q. No: 2)

AGE PROOF

       Standard Age Proof:

       Birth certificate, School/collage Certificate, Bonafide Certificate, Pass Port, PAN Card,                       Employer Certificate, Certificate of Baptism, Domicile Certificate. Driving License

       Non Standard Age Proof:

      Self Declaration with annexture-1 & extra premium for limited SA
